How To Use This FacilityDownload and complete the form below. Left click the form for the image to open and then right click the image and copy the image. You can paste this image in any program like Microsoft Word. Print the form at your end or save in any CD or pen-drive and get it printed.
Enclosures To The Application:
1. Five photographs (size approx. 2.5 cms by 3.0 cms and not larger than that); it would be desirable to affix a hair from the behind each photograph and to put your name in your own handwriting at the back/front side of each photograph.
2. Remittance. In case of your personal visit to the centre you can deposit cash, or else the application should be accompanied by a cheque or bank draft, favouring 'Deepak Kashyap', payable at Agra, India.
3. In case you feel you would like to use the facility for more than one dates, you can remit the amount for more than one date. The Healing would be given on the date(s) of your choice mentioned in your application.
